The Mobile Health Care System (MHCS), Community Check program (CCP) offers medical care that allows the participant to stay in the comfort of their own home.  The goal is to reduce risk of injury and illnesses that can be prevented as well as to have fewer visits to the emergency room or doctor’s office.  

A trained provider (EMT or Paramedic) will come to the participants home, and do an evaluation. The providers are in immediate contact with the participant’s doctor should anything be out of the parameters set by the physician.  

This program is provided free of cost to the participant. The CCP is made possible through funding from the Adirondack Health Institute Innovations Grant as well as a collaboration between Cambridge Valley, Bay Ridge, Easton Greenwich, Granville, and Salem Rescue Squads, Alzheimer’s Association, Medical Directors, Local Health Centers, Hospitals, Regional Health Organizations and Washington County Public Health.
